Getting The Google Text Banner Position

Occasionally you'll see that some paid listings on Google appear above search results, in a space once occupied by the old CPM-based "premium listings" or what were once called by Google to be "text banners." How do AdWords listings end up in this premium spot? AdWordsRep, Google's always busy official rep for AdWords on our forums, provides information in this thread, Top position? Impossible?
